Florida KidCare, public health benefits for uninsured kids 18 and under, is available through the state. Applications are being accepted throughout the year.

You will need Adobe Acrobat Reader to print the application. You can get Adobe Acrobat Reader for your computer here.

The application and instructions are a total of 5 pages, formatted for legal-size paper (8 1/2" x 14").

Please print the application on legal-size paper (8 1/2" x 14"). Do not use letter-size paper or reduce (shrink) the size of the application because it makes the print so small that data-entry errors could occur and cause delays in determining your child's eligibility.
